The Department of Clinical Oncology (hereafter the “Department”) complies with all laws and regulations regarding personal information, and takes all possible measures to protect patients’ valuable personal information.
Collection of Personal Information
The Department may collect personal information, to the extent necessary, in the following cases:
- When contacting the Department with an inquiry
- When making an appointment with the Department, requesting a second opinion from the Department, etc.
Purposes of Use of Personal Information
The Department uses personal information collected from patients for the following purposes:
- To contact patients
- To answer inquiries from patients
- To provide services to patients
- For education and clinical research (personal information is anonymized before use)
Provision of Personal Information to Third Parties
The Department does not disclose or provide personal information collected from patients to third parties, with the following exceptions:
- When the patient has consented to doing so
- When there is a request from a public office, such as the police
- When the law applies
Disclosure, Correction, etc. of Personal Information
If the Department receives a request from a patient regarding disclosure, correction, deletion, etc. of that patient’s personal information, the Department shall comply.
